When an Auditor Says "Show Me," How Fast Can You Answer?
The question is not whether the record exists. It is how long it takes to prove it.
Most RTOs can eventually answer an auditor's question. The useful measure is how long it takes, and how confident you are in the answer.
Slow answers usually point to the same causes: records spread across several systems, context stored in someone's inbox, and changes nobody can trace.
Try this
Pick one record from six months ago. Ask a colleague to explain what happened to it, who changed it, and why. Time them.
If the answer takes a day of searching, the problem is not the audit. It is that your organisation's history lives in people's heads.
